It is easy to sign your school up for the 2010 Foodbank WA School Breakfast Program!
There are 3 sections necessary for your application:
- Complete the 2010 Foodbank WA School Breakfast Program Registration form
- Complete the 2010 Foodbank WA Agreement form
- Attach a Letter of Support

Foodbank WA School Breakfast Program Registration Form
Please complete all information requested on the 2010 Registration, and return the completed form (minus this instruction page) with your application.
The Registration form must be completed in full – incomplete forms will be returned for completion.

Foodbank WA Agreement Form
This form needs to be read and signed by the School Principal. Please complete & submit the following pages of the Agreement form with your application:
- Page 1: ‘Agency Name’ – Just complete the schools name, as contact details will be on your Registration Form. Please note that schools do not need to complete the “Registered Charity No”
- Page 4: ‘Persons authorised to collect food’ – Card holder details are essential as this indicates who has permission to withdraw product from Foodbank WA on behalf of your school (up to 4 card-holders).
- Page 6: ‘Signature’ – The school principal must sign the agreement for the application to be processed. Please note that schools do not need to complete the “Agency Registration” box

Letter of Support
This is a brief letter outlining the demonstrated needs within the school community that that have prompted you to establish / continue a school breakfast program. Each school is assessed on its own merit, so it is imperative to include this letter.
Please complete both forms together with your letter of support and forward them to the Foodbank WA Perth office:

Please retain a copy of your application.
Please allow up to a week for processing. Upon approval, the principal & coordinator will be advised (via email) of successful registration & supplied with the appropriate order form. Nominated card holder/s will receive their individual cards on their first visit to Foodbank WA. No identification is required as signatures will be verified by those supplied on page 4 of the 2010 Foodbank WA Agreement Form.
